You will probably better off contracting out an experienced DBA to do the techie part of the interview for you. Trying to get a feel for this type of thing with a prewritten checklist is probably not going to allow you to find someone good. Believe me, I've been doing several DBA interviews lately and it would be difficult to tell with some people if I didn't do the job myself. Good luck!
