I don't know of any problems it causes (except for the extra effort when we trashed a disk which was only part of a logical volume.) Since we began using LVM with Online from the start I have no idea if there is a perfomance problem. As far a I can tell Online manages the Logical Volume in exactly the same way it would a physical volume.
