I would bet that it has something to do with this. Maybe it would be worth trying to output your PATH setting in the beginning of your CGI script and hence observing in what context it actually runs. The application requires 16-bit CLI! IIS4 is running as a process (not a service) and uses the (16-bit-) application via OLE.
