Jonathan, Are you using any other cursors in your program? You say this effect is random? By that I presume it works for a number of times and then fails. Has it ever failed first time? Are you using Key-Only Select techniques? If not, then I think you should consider it. I would also suggest that you should check the logic to make sure that you don't re-open any of the cursors without closing them.
