I am happy to reply back to the group that the apparent reason for
-211 (matched with -101 ISAM) has been determined for our
application. Since one of our larger tables was "fragmented" it was
decided to turn on PDQ (Parallel Database Queries). This option is
described here:

I'm sure this option works most of the time and probably helps
performance-wise in many applications, BUT... for us there is a direct
correspondency between this option being on and our application
receiving a -211 SQLCODE matched with -101 ISAM ERROR. We ran several
thoursand of our transactions and were able to demonstrate the
connection between this setting and getting the error. Fortunately
for us using PDQ didn't seem to improve performance of our
application.
Hopefully this will help others who may have experienced this problem.
