I've come across some really strange behaviour related to the use of
UPPER in a where clause.
For grins and giggles, take a peak at the explain output, and see if all
the pieces look right - like picking the right index, etc. (ie. a really
close look). I've found that UPPER can cause the
engine to pick the wrong index, and get really confused. Try playing
with it without the upper, and see what happens....
