Nope, the 28 was encountered when trying to write shared memory to a file in
/tmp. It is simply letting you know that it did not actually dump shared
memory to the file. This is not the cause of the segmentation violation.
Unfortunately, I don't have anything useful to say about the segv, except
that you are on an old version of OnLine, and should consider upgrading.
