It's a bug that's been there since time immemorial, the causes varied with the versions if I recall correctly, but the result was the same - once you had more than N forms or reports or SQL scripts or whatever, some of them would randomly disappear from the pick lists. You don't mention which version of ISQL you're running - it might be helpful.
