errno=6 is 'no such device or address', maybe the kernel isn't configured
right. Try doing something like
dd if=/dev/zero of=/dev/rdsk/c21t5d1s1 count=1000 bs=2k
being VERY CAREFUL to get the disk name right (or you might wipe out
a Unix partition :-) and see if that works. It and informix are doing
about the same thing, but this takes informix out of the loop so you can
bug NCR tech support if it fails, check your config, etc.
Another thing to check is the permissions -- online 5.0 at least doesn't
install quite right for our (DG) version of SVR4 -- namely the owner
and mode of the devs needs to be reset at each reboot via the rc script.
Setting it once only is no good, since /dev is rebuilt at reboot time.
But that should generate a permission denied error, so probably not your
problem here.
