My first self-inflicted disaster was far duller:
# rm -rf / tmp/filepattern
yes, that's a root prompt, and yes, my fingers slipped a space in there, and
it did a lot of damage...

My equivalent had someone else in the driving seat, but nice new machine, lots of software installed. Created a temporary user for some reason - so put their home directory as root, /, since there really wasn't a need to create a directory just for them. Nice script to remove a user - asks if you want to remove the home directory too. The script didn't check whether that was / and did a very good job of 'rm -fr /'. It is surprising how long a machine can continue to run after you've lobotomized it.
