Long ago I reported a bcheck bug regarding duplicate values. If there were more than 65535 duplicate index key values, bcheck would always say that the index was out of order. Running bcheck would "fix" it, but it would still call it out of order. Do you have a lot of duplicates? If so, maybe that bug is still in in your version.
