And did you?
Could be that you're trying to bring up too big an instance, or that a
previous instance wasn't cleared from memory, or that there is an instance
already at that address.
Try running onmode -ky a couple of times before retrying the oninit, or
reboot the server.

You will find, in most cases, very useful machine/platform specific
information in the text files in your $INFORMIXDIR/release directory.
In addition to what Paul said about /etc/system, check your informix
machine or server notes in the release directory for the needed
/etc/system changes/additions. In addition to what Obnoxio said, check
your ipcs after such a failure and make sure your memory segments are
cleaned.
Check the release directory for information, such as is "SHMBASE
0xa000000" the right shmbase for your setup? I just checked, you are
probably safe on that or you might get a "shmat: [EINVAL][22]: shared
memory base address illegal" error.

Although the link '/informix/chunk/rootchunk' has ownership informix:informix
this may not be true of the underlying raw space '/dev/rdsk/c0t2d0s1' (or
indeed '/dev/rdsk/c0t2d0s3'). It is these that need to be owned as
informix:informix and have permissions of 660 the ownership of the links is
irrelevant.

The message "mt_shm_remove: WARNING: may not have removed all/correct
segment" in log can refers a shared memory segment not removed after
initialize not properly. Then: reboot the box (you can see ipcs -m and
ipcrm if you don't want reboot), change owner-ID and group-ID for
/dev/rdsk/c0t2d0s1 and reinitilize rootdbs.
