My understanding is that the behaviour is different in 9.4 than previous versions. It seems to take a lot more space than 7.31, but that may be just my experience. First extent size for an index is estimated using a function of first extent for the table. Of course, if that extent is filled and the next allocated extent is contiguous, it will join the extents as per tables. We had some serious extent issues when the first extent size was left to default 8 pages but the table had grown very, very large, with many unique values, and this, we were told is the expected behaviour. It matched what we saw, but was not what we expected.
