I have had this sort of thing happen to me, in fact it has just happened
again.
For one of ours, it was explain by a null being passed as a variable in
SQL within 4GL. This went something like:
SELECT col1,col2
FROM tab
WHERE col2 = variable
variable is supposed to be set by the application from a value keyed
into a screen. What we found was that if the user didn't key anything,
and we didn't vet the input before we performed the SQL, the server
crashed in the manner you described. Unfortunately I cannot remember
the stack trace for that one :-(
