For all Read Next errors there is an ISAM error which you need to determine
why it failed. It is almost certainly an application (indexing) issue...Run
that sql with set explain on and determine whether you have the correct
indexing strategy.
You may want to set lock mode to wait, so if you strike a lock your
application does not die.

Bill,
As Murry pointed out that its an application issue, also check if
exclusive lock has been placed by any process on req_line.
