Shared memory issue

I need help on this -- After Informix was shutdown with "onmode -ky" and started with "oninit", I got error "out of shared memory". I checked the system with "ipcs" command and there is nothing. The problem is re-occuring every week. After we rebooted the system, the DB crashed. Anybody has idea what could go wrong? The system was running fine for 2 years with the same procedure. No configuration parameter has being chenged. We just add 100GB new disks for 16 new dbspaces. Jean Wang
