I just received a somewhat similar error on HP-UX 11.11 7.31.fd5.
22:03:04 Exception Recursion while handling: MT_EX_OS, Context: mem
03:28:38 VP Notify mechanism incomplete after 5 minutes. This can bedue to slow network file access. Will try 12 more times
After contacting support, they determined an AIO VP was in a running
state from which it wasn't exiting. I restarted the informix server and
problem went away.
