It relies on paranoia and a general misconception of how virii and Email work. Point one: Virus check all files drawn off the net. The most popular vector for transmission of Virii is via executable files, the second via zipped executables. There are rumours that uncertificated javascript and directX can be made to 'do things' but this is largely rumour. Y'see the problem is that once people read the message talking about 'Irina', then they delete further warnings on 'Irina'. It's one of the longest running chain-letters around, and a rather clever take on Dawkins 'Memes' in the regard of ideas being semi-living things.
