Triggers not always work???

Online 7.11. For some reason triggers are not always getting invoked by an esql/c update program. My understanding was that there was no way around the trigger - it resided at an engine level and ANY update to the table would invoke the trigger. This is apparently not the case. Thoughts? There IS an update and insert trigger, and they work fine from all other places.
