Requirements -> DB Model

I'm, again, in the process of designing a database for large project and thinking its about time to formalize the process. Basically, I'm going to write out a step-by-step list of the tasks that I need to do to go from requirements to a physical database model. So does anyone happen to know what 1) documentation is available on the net, 2) a *really* good book and/or 3) anything other thoughts on the subject that you think might help me out.
