Another thing I found on a recent training course is that if you have two separate SQLHOSTS files which use the same value in the "services" column on a shared memory entry, but they actually point to two different instances, you get this problem. So, not only do your entries for different instances have to be unique within an SQLHOSTS file, they also need to be unique across SQLHOSTS files.
