Carroll > GTE Wireless > The most obvious one is the chunk is no longer there, I would use dd to check it ie dd if="path to chunk", if this errors the disk has probably gone on its holidays. Depending on the flavour of unix you might have utilities such as prtdiag/errpt etc. If the disk has gone then put a new disk in and then re-establish the link and either resync from the other half of the mirror or restore from tape.
