Pawel,
I'll give it to you straight - you're in trouble. It appears that
your logical log file is corrupt. That's the iserrno 126 you're
seeing. (ISAM Error 126 = Bad Row Id being referrenced) Without
being able to process your logical logs, the recovery process will
not be able to restore your database to logical consistency.
(Specifically, it looks like it's unable to rollback an uncommitted
transaction. Most likely the one which failed due to too few locks.)
It looks like you've just been very unlucky. There doesn't
appear to be anything wrong with your configuration aside from
(perhaps) a shortage of locks. There's nothing to be done about
that now though.
Your best bet is to restore from an archive tape. If you also
had continuous logging on, then you should be in terrific shape.
Without continuous logging, you're in ok shape. Without an
archive, you're in <gulp> terrible shape.
Good luck! Also, please feel free to e-mail me if you have
other questions.
