List of All Indexes

I am trying to compose a single SQL query which will list all table names, index names, and the columns on which those indexes were created. For example, say I had created the index "customer" using the following query: create index customer on customer_list(custnumber,lastname); When I run the SQL query, I'd like to see something like: customer_list customer custnumber lastname I've tried a few variations using sysindexes, systables, and syscolumns, but I can't get the column names to come up in the right order.
