Update not updating

On our production box, an HP9000 with version 7 of Informix, we have a situation where after a program attempts to do an insert into a table and discovers the record already exists, the program attempts to do an update. The update returns a sqlcode of zero and a sqlerrd record count of zero records updated. One record should have been updated. We cant seem to duplicate the situation on our development box, also an HP9000 version 7 of Informix. This doesn't seem to be a commit problem. Any ideas?
