The kernel has been tuned/changed as per the release notes, using the ipcs -m, no shared memory segments are showing up out there, and the initialization never gets far enough to write to the online.log.
Anyone have any ideas, or have had any similar problems?

Try running onmode -ky a couple of times before running oninit. You might
have some shared memory hanging about. Anything in your online.log?
