Question: Shared Mem Init problem

Hello, it would make my week if someone could tell me what is going
on and how to fix it. I have 6.0 running on a Sparcstation2. When
booting, I get the following error messages on the console:
oninit: cannot open chunk '/dev/rsd16h' see online message log
oninit: fatal error in shared memory initialization
The online message log goes like this:
Mon Nov 28 09:41:27 1994
09:41:27 warning: chunk timestamps are invalid
09:41:28 I/O error, Primary Chunk '/dev/rsd16h' -- Offline (sanity)
09:41:28 INFORMIX-OnLine Stopped
09:41:29 mt_shm_remove: WARNING: may not have removed all/correct segments
I have looked into using SunSolve to find the answer, but the amount of info
about chunks and shared memory and timestamps is enormous. I could see looking
through it for days and not finding a solution.
Has anyone run into this before? Am I missing something obvious?
Thanks in advance for your suggestions/solutions! -- Alan
BTW. I have seen the thread about putting an offline chunk back online with
hex editor, but tbcheck -pr does not work when shared memory init failed.
