You usually gets these (and/or some others) when you try to access a database instance on another machine, or use synonyms which do the same. The fix is to set up any remote users in the local password file and possibly add entries to the hosts.equiv file. If you're not trying to let remote users to your database, or trying to get to a remote database, you can also get the errors on a local machine if you're using a network connection type (not ?pipes? on SE) and the username supplied is not in the local password file. I've also seen hosts.equiv come into play even in these instances (can't remember the details).
