c-isam datablade server storage questions

The User's Guide (Version 1.0 December 1999 Part No 000-6498) says page 4-6: "Recompile and link your c-isam application with the libraries provided with the C-ISAM datablade module." Question: is recompile needed or can we just do the linking step (f.ex. when only access to library of application doing c-isam calls is possible, no source being available). When a c-isam file is migrated to a database table, a table layout has to be choosen. If for applications needs, an index (assume char data) has to be added later and the table design does not allow to find mapping columns, how to handle this in server-express? In c-isam there would be no problem as indexes are defined on physical entities, offsets in data record with a length to build the composing parts of the new index.
