A couple things to try (if you haven't already):
1) Try the very same select statement(s) in dbaccess to make sure you
don't have a join that is causing a row to be selected more than once.
B) Replace the FOREACH with a specific loop and see if duplicate rows
still come through (this will confirm that the problem is in the code,
not in the data).
iii) See that improper use of a group or page header isn't causing the
problem. Comment out all header blocks (keep only ON EVERY ROW if
possible).
$) How many times is OUTPUT TO REPORT called? Don't assume once because
the line of code appears only once. With looping, etc. maybe you're
calling it more than you think.
LAST) Send me a copy of the report and I'll see if I see anything
obvious.
