insert into temp w/ union

I'm using ACE to create a report but to get the data, I first have to make a temp table holding two dates. I can't use 'create temp table...', I have to use 'select... into temp'. The select into temp statement has a union that contails 13 different column names. How can I name the column?? I've tried 'select field1 date1, field2 date2... union select field3 date1, field3 date2...' and so on, but in the SQL that I use to collect the data, it says date1 and date2 don't exist in the table. What do I need to do?
