You can build an index on an empty table. It simply creates the root node,
and puts entries into your system catalogs and tblspace tblspace indicating
that the index exists. Then each time you add a row, it also adds an index
entry for the row. That's what's meant by "building an index first, then
loading the table." We've already covered the fact that this is not what
happens in dbimport, however it certainly can be done.
