Stating the obvious - but have you compiled the form ?
Form files are found using DBPATH, so as long as there is a the .frm somewhere
in that path (or the current directory) - it should be picked up...

Your form is not in the current directory. A solution, is include the full
path in the especification of the OPEN FORM statement.

Is DBPATH set? If not, include the full path to the
form file as part of DBPATH.
Let's say the form is located in the /usr2/infxforms directory. Include this
directory as part of DBPATH.
