I have a customer with four servers with identical software on each.
Without any new code being introduced, we started getting -211 errors
last Friday.
Online log is not reporting anything and I cannot find any locks being
held (when the ISAM error was being reported, it was concerning a lock).
It has been a long, long time since I ran into one of these errors. Any
ideas?

I'd start by running oncheck -cc <database>. After that I'd consider trapping
for the error with onmode -I 211 (onmode -i will unset the error trap once the
af file is generated)and looking at the diagnostics. I think the onmode -I cmd
is undocumented so you might wanna call support if oncheck -cc doesn't reveal
anything.
