Sounds like you are either out of memory or don't have enough SHM handlers to cope with the additional segment requests. You need to check the SHM settings in the kernel and/or increase your initial SHM parameters. You could also be unintentionally looping in your code causing the memory to increased. Can you post the insertion code?
