Judging from the code in cgigetenv.c, which is called from cgistart.c, which in turn is called from icgistart.c, which is where icgi_start() is defined, then I would guess that one or other of the environment variables SERVER_NAME or REQUEST_METHOD is not set correctly. See also the code in cgimethod.c. icgi_start() does not do much, if anything, with the database itself.
