As Ben points out in his response, for to set up a new instance, as after a
new installation, you need to also initialize disk structures. Make sure
that the disk file named in ROOTDBS in the ONCONFIG file exists and add the
'-iy' flags to the oninit startup. VERY IMPORTANT the '-i' flag will wipe
out the contents of the ROOTDBS (making any existing data inaccessible) so
you ONLY want to do this once at the first startup of a new instance on a
machine. From then on the '-i' MUST be removed.
