online error

Hi,
Our informix online (version 5.03) crashed with the
following online.log output. The uid, 550, was
user informix. I can't track down the pid at this point.
After consulting the administrators guide, I
re-initialized the shared memory and then brought the
system back on line. I am not clear on WHY this error
occured, though, and would like to track it down as
we have a real-time system that relies on the
database.
Also, recently the permissions had been changed on
the link to the raw device that is our primary
chunk. We just changed the permissions and the system
appeared to come up cleanly. FWIW.
We have recently upgraded our operating system on
the database machine to HP-UX A.09.05 A and are running
on a 9000/715. It seems that the problems may be
related, but wanted to get some input on possible
causes before I start procedures to upgrade informix
or back out the OS upgrade.
