Serious oninit -i problem?

oninit -i produced the following lines in the online.log on Solaris 2.7The bit about the unusable chunk doesn't look healthy to me!
BTW, rootdbs is a symbolic link to a raw meta device
i.e. /dev/md/rdsk/d30
Any ideas?

What does /tmp/af.3f3188b say?
This should tell you what the problem is..
