While your screen array has to match your program array, your program array does NOT have to match your table! So, define your program array to hold the six columns the user needs to fiddle with. Then, match your screen array with the program array. During the insert, have a record like table.* structure which is used for the actual insert statement. Populate it partly with your program array. Populate the remaining fields(columns) with, presumably, your audit information (createtime, user, etc). Then, insert away.
