PERFORM - unknown message 3004

Does anyone have any information on "unknown message 3004" from PERFORM? As advertised, this message is unknown to the writers of the error list in the manual. I get this response when I try to run a form within the ISQL environment and when I try to invoke sperform from the command line. I generated and compiled the form in the environment, so it's not that I'm trying to run a FORMS4GL form or some other silliness. I'm running ISQL version 2.10.03 (yes, I know it's ancient).
