possible causes of -211 SQL ERROR

-211 Cannot read system catalog catalog-table. I know you're supposed to see the ISAM error, but some bastard was not getting it correctly, so now I'm very curious as to what caused it. (It was during an update statement). I ran tbcheck afterwards on the table in question and it looked ok, so I assumed one of the system tables must have been locked, until I asked myself why a system table might be locked and the only explanation I could think of was if someone were doing some sort of alter table or index or create or drop or something like that and noone with that authority was anywherenear the machine.
