Index failure problem

Hi,
I work on Informix 7.3 and sometimes I have problems with indexes that I
have created by mysef, they where not created automaticly during the
creation of foregin key or unique constraint. For example I have created
index for nvarchar(20) column in the table to speed up the queries. For that
index sometimes I have an index failure error in the IECC Console. I also
use the indexes on tempopery tables and they sometimes cause this problem.
I would like to know if I can avoid this. I create indexes like that:
create index index_name on table_name(column);

What is the 'index failure' error message?

Is you database logged? I see this frequently with unlogged databases!
