This message is an indication that the server crashed and didn't tidy up
behind itself properly. onmode -ky should clear out anything left behind,
failing which you can use ipcs to identify the shared memory segments and
ipcrm to get rid of them. Or reboot.
If you still have a problem after that, we'll probably need about another
20-odd lines of your log to see the problem. In fact, cat /dev/null to your
log file, and then reproduce the problem. Then send us the whole log.
