OK, let's take another look at the original error message.
It suggests that I read too fast and not accurately enough.

This means that OnMonitor tried to display some menu in a window which
probably had a form in it. Is there any chance that your DBPATH points to
a directory which also contains a form file with the same name as one of
the OnLine forms (whose names you can find in or under $INFORMIXDIR/forms).

If so, unset your DBPATH, or put $INFORMIXDIR/forms in front of it. We
might need to work out exactly which menu is not displaying. Which Unix
platform are you on, and which version of OnLine/OnMonitor are you using?
If it isn't something to do with this, then we'll need to do some horribly
detailed tracking. Does OnMonitor work if you login directly to the
machine?
